Nursing Team
Nurses have a critical role in the pre-op and post-op care of our joint replacement patients that is so important for successful outcomes. The Pre-Op Nursing team contacts patients well before their surgery to gather information about their health status and medications that help the Joint Care Team provide personalized patient care.Upon admission, the Bedside Nursing and Social Services Team gathers information from patients and family members (coaches) to determine what resources are available at home and help patients and families establish realistic goals for being discharged directly home or to a short-term rehab facility.
After surgery, nurses reinforce what the patient and coaches learned during their pre-op education and focus on patients’ comfort and effective pain management which are vital for getting patients up and moving. Upon discharge, nurses provide further instructions and education so patients and coaches have the equipment, knowledge and confidence to continue safe recuperation at home.
Social Services
Our licensed clinical social workers help assess patients to determine the best discharge plan. While a high percentage of our joint replacement patients are discharged home, some will need to go to short-term rehab facility. Social workers arrange for that continuation of care.
