KishHealth System and dekalb county health department announce a county-wide online community health needs assessment survey

KishHealth System, in collaboration with DeKalb County Health Department announced today that it began an online survey on August 30, 2012. This online survey is an integral part of the community health needs assessment conducted once every three years by Kishwaukee Community Hospital and Valley West Community Hospital and once every five years by the DeKalb County Health Department. Results of this survey will be combined with existing federal, state and local health data. The results of the community health needs assessment will provide guidance to identify health problems and fulfill both federal and state mandates.
Participation is voluntary and the online survey is designed to measure residents’ perceptions of health care delivery, access and health practices. The confidential online survey uses standardized questions, is available in English and Spanish and will take approximately 15-20 minutes to complete. Willingness of DeKalb County residents to participate will support the hospitals, the health department and other community agencies in the development of vital programs and services which will advance the promotion of health.
The community health needs assessment process ensures the Kishwaukee Community Hospital, Valley West Community Hospital and DeKalb County Health Department are responsive to their communities by understanding and acting to address priority health needs.
To access the online survey, participants are asked to visit www.kishhealth.org/communitysurvey. A report outlining areas of health needs will be published on the Kishwaukee Community Hospital and Valley West Community Hospital websites and the DeKalb County Health Department website in late December 2012.
