Empower Program
The Empower Program works with patients on their journey towards recovery by offering education and support during and after their hospital stay, with the goal of preventing frequent, preventable and costly readmissions to the hospital.
The program aims to take hospital services beyond patient care, equipping patients for an easy transition from the hospital to home or another care facility. The Empower program staff work individually with patients to be sure they understand their diagnosis, treatment and any necessary self-management skills to improve and maintain quality of life. Assistance may include making follow-up appointments for patients, notifying a pharmacy of additional education needs for prescriptions, and consulting with local community and social service programs when beneficial.
The Empower program staff will follow-up with patients after their hospital discharge to ensure their needs are being met. This service allows patients and their families to ask questions, address concerns and find resources for additional support to aid in recovery and ongoing wellness.
